Is this debt collection notice real? US Bank claims we owe $3K from 2006

Can someone verify a document for me? I received a call about serving papers to my husband. They later sent a form claiming he owes US BANK $3,000, with a $1,600 settlement offer or a $10,000 court threat. He recalls no account with them. They stated the account opened in 2006—nearly 20 years ago. I question its validity because the paperwork lists old addresses, his social security nummber, his work, and his Harley and bank loans. I will message a redacted picture of the document to anyone who can help. Google shows no results for the phone numbers. US bank found no account under his name.
 
Man, this just feels like a total scam, you know? (Just my two cents, not an expert).
If the bank was really about to take him to court, they'd definitely have some proof he was a customer.
